Job summary

Amity Foundation in Arizona is seeking a compassionate Care Coordinator to support students at Circle Tree Ranch. You will coordinate services, conduct assessments, monitor treatment plans, and advocate for student needs to promote holistic well-being.

This full-time, on-site role requires crisis intervention, strong communication, and documentation skills. You will collaborate with clinical staff, external agencies, and the DCS to ensure safe transitions and access to resources.

Qualifications

• 3–5 years' experience in behavioral health case management
• Experience with Therapeutic Teaching Community concepts is a plus
• Strong written and verbal communication; confidential information handling

Responsibilities

• Coordinate meetings with recovery team and health homes, probation officers, and other parties to support student needs
• Develop and implement transition plans addressing housing, employment, education, and ongoing support
• Communicate with the Department of Child Safety (DCS) to support students in the child welfare system
• Assist students with court-related matters, including hearings and documentation
• Help students obtain identification and other essential documents for employment, housing, and services
