Job summary

Catholic Charities Comm in Cottonwood, AZ seeks a Recovery Coach/Substance Abuse Case Coordinator to mentor and advocate for individuals in recovery. You will provide outreach, education, and comprehensive case management through the AFF program, coordinating services and supporting families affected by substance use.

Role emphasizes collaboration with an interdisciplinary team, documentation, and ongoing client advocacy.

Qualifications

• Associate's degree in human services or behavioral health, or HS diploma with 3+ years related experience.
• Paraprofessional certification within 90 days or recovery coach certification.
• Must be substance-free for at least six months following treatment.
• Must have valid AZ driver’s license and reliable transportation.
• Must pass Level One Fingerprint clearance.

Responsibilities

• Provide 24-hour in person outreach to families referred by DCS.
• Provide recovery education for every phase of recovery.
• Be a model by demonstrating that recovery is possible.
• Help clients identify personal interests, goals and strengths.
• Assist clients to develop their own recovery plan.
• Promote self-advocacy and client voice in rehabilitation.
• Participate in interdisciplinary team meetings as needed.
• Oversee collection and review of collateral information and releases.
• Provide ongoing support and case management to clients.
• Develop individualized service plans and update quarterly or more often.
• Submit referrals for Substance Abuse Assessment and coordinate meetings.
• Coordinate services and provide information about community resources.
• Maintain accurate documentation and billing in agency systems.
• Contribute to quality improvement and service excellence.
