Job summary

iCare Health Services, LLC is seeking a Crisis Case Manager / Care Coordinator to support our Behavioral Health Crisis Stabilization and 23-Hour Observation teams in Glendale, AZ. You will coordinate care across providers, arrange discharge plans, navigate community resources, and facilitate warm handoffs.

The role requires strong communication, organized documentation, and the ability to work with hospitals, clinicians, and families to ensure safe transitions for individuals in crisis.

Qualifications

• Bachelor's degree in social work, psychology, behavioral health, human services, counseling, sociology, criminal justice, or a related field preferred.
• Behavioral health, case management, care coordination, social services, or related experience strongly preferred.
• Experience working with individuals with mental health and/or substance-use conditions.

Responsibilities

• Provide rapid care coordination and discharge planning.
• Identify barriers to safe discharge (housing, transportation, food, medications, insurance, benefits).
• Participate in multidisciplinary treatment and discharge-planning meetings.
• Develop and coordinate individualized transition and aftercare plans with the clinical team.
