Job summary

Amity Foundation in Tucson, AZ, seeks a dedicated Care Coordinator for Circle Tree Ranch campus to support students transitioning back into the community. You will coordinate services, monitor treatment plans, and advocate for student needs within a holistic care framework.

The role is full-time, on-site, hourly, and involves collaboration with clinical staff and external agencies, plus documentation and transition planning for housing, employment, and education.

Qualifications

• High School diploma or equivalent.
• 3–5 years' experience in behavioral health case management.
• Experience with Therapeutic Teaching Community fundamentals.
• Arizona DPS Fingerprint Clearance card.
• CPR and First Aid certification.
• CPI & ESR certifications.
• Proficiency with Microsoft Office Suite and EMR systems.
• Excellent organizational and written/verbal communication skills.
• Ability to maintain confidential information.
• Empathy, active listening, and diversity-sensitive interactions.

Responsibilities

• Coordinates and facilitates meetings with the student’s recovery team and external parties.
• Develops and implements transition plans addressing housing, employment, and education.
• Collaborates with Department of Child Safety to support students in care.
• Assists students with court-related matters and documentation.
• Supports students in obtaining IDs and essential documents for work and housing.
