Job summary

Responsibilities

Create and maintain a safe, supportive home environment focused on individual goals.
Model, support, and supervise Direct Support Professionals in daily activities.
Be available on-call and serve as DSP when needed.
Coordinate staff scheduling and ensure coverage for shifts and activities.
Attend appointments with individuals and manage timelines and follow-ups.
Submit medication changes and ensure MARs are up to date monthly.
Lead data collection, progress tracking, and documentation accuracy.
Collaborate with DPM/HR on hiring, training, and performance feedback.
