Qualifikationen

• CA-licensed OT as required.
• OT program design and implementation experience.
• Experience collaborating with interdisciplinary teams advantageous.

Aufgaben

• Design and implement an OT program to help participants maintain independence.
• Oversee execution of care plans and perform reassessments as needed.
• Collaborate with the IDT across Day Center, PACE Center, nursing homes and in-home settings.
• Assist in ordering, inventory and maintenance of durable medical equipment.
