St. Paul’s Senior Services in California invites a Licensed Occupational Therapist to join the Paul’s PACE program on an on-call basis.

You will design and implement an OT program to help participants aged 55+ with chronic illness or disabilities maintain independence and safety at home and in the community. As a key member of the interdisciplinary team, you’ll oversee care plans, perform reassessments, coordinate across Day Center, PACE Center, nursing homes and in-home visits, and assist with

Qualifications

• Licensed Occupational Therapist in California.
• Graduate of an OT curriculum accredited by relevant bodies.

Responsibilities

• Design and implement an OT program to help participants maintain independence.
• Oversee execution of care plans and perform reassessments.
• Coordinate with the IDT across settings (Day Center, PACE Center, nursing homes, in-home).
• Assist in ordering, inventory and maintenance of durable medical equipment.
