Saint Therese is seeking an on-call MDS Coordinator at our Oxbow Lake campus in Brooklyn Park, MN. You will lead the MDS/RAI assessment process to ensure regulatory readiness, accurate documentation, and high-quality resident care across interdisciplinary teams.

Qualifications

• Current licensure as RN or LPN in good standing.
• Minimum of three years in long-term care.
• Experience with MDS/RAI processes and Case Mix.
• Experience with PPS, Medicare, HMOs, managed care, and third-party payors.
• Experience with care planning in long-term care.
• Strong understanding of clinical assessments, documentation, and regulatory requirements.
• Proficiency with Microsoft Office, EHR, ROX, and Excel.
• Strong written and verbal communication skills.
• Ability to work independently and in teams.
• Excellent organizational and time management skills.
• High professionalism and attention to detail.

Responsibilities

• Coordinate and complete the full RAI/MDS process for assigned residents.
• Ensure assessments are timely and compliant with regulations.
• Conduct resident interviews and participate in care conferences and discharge meetings.
• Apply knowledge of Case Mix, PPS, Medicare, HMOs, managed care, and third-party payors for reimbursement.
• Utilize ICD-9/ICD-10 coding for documentation and care planning.
• Review charts and information to identify concerns and elevate appropriately.
• Partner with interdisciplinary teams to address quality of care opportunities.
• Support development of individualized care plans based on assessments.
• Contribute to regulatory readiness, quality outcomes, and resident satisfaction.
• Maintain records and reporting using Microsoft Office, EHR systems, ROX, and Excel.
• Prioritize workload to meet regulatory and organizational timelines.
• Support consistent workflows and documentation standards across the team.
