The role requires coordinating MDS assessments, collaborating with the care team, and maintaining compliance with CMS and Minnesota regulations. The ideal candidate holds an MN RN license with 1–2 years of MDS experience, strong RAI knowledge, and experience in SNF/long-term care.

Qualifications

• Current RN license in Minnesota.
• 1–2 years MDS experience preferred.
• Strong knowledge of RAI, ICD-10, and PDPM.
• Experience in a skilled nursing facility (SNF) or long-term care setting.
• Strong organizational skills and attention to detail.
• Excellent communication and teamwork abilities.

Responsibilities

• Coordinate and complete MDS assessments (OBRA, PPS, state-specific).
• Ensure timely, accurate MDS documentation and submissions.
• Collaborate with interdisciplinary team to develop care plans.
• Monitor case mix index (CMI) and identify optimization opportunities.
• Ensure compliance with CMS guidelines and Minnesota regulations.
• Participate in care plan meetings with residents and families.
• Assist with audits, surveys, and quality improvement initiatives.
• Stay up to date on regulatory changes and reimbursement updates.

Position Summary

Southview Acres Healthcare Center is seeking a detail-oriented and experienced MDS Coordinator to join our clinical leadership team. The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations, ensuring accurate reimbursement, and supporting high-quality resident care.

Key Responsibilities

• Coordinate and complete MDS assessments (including OBRA, PPS, and any applicable state-specific assessments)
• Ensure accuracy and timeliness of all MDS documentation and submissions
• Collaborate with interdisciplinary team members to develop and update comprehensive care plans
• Monitor case mix index (CMI) and identify opportunities for optimization
• Ensure compliance with CMS guidelines and Minnesota state regulations
• Participate in care plan meetings and communicate effectively with residents and families
• Assist with audits, surveys, and quality improvement initiatives
• Stay up to date on regulatory changes and reimbursement updates
