Superior Healthcare Management's Southview Acres Healthcare Center in St. Paul, MN is seeking an experienced MDS Coordinator (RN) to join the clinical leadership team. You will oversee the RAI process, ensure accurate reimbursement, and support high-quality resident care.

The ideal candidate holds a Minnesota RN license, has 1–2 years of MDS experience, and demonstrates strong knowledge of RAI, ICD-10, and PDPM. Excellent organizational and communication skills are essential.

Qualifications

• Current RN license in Minnesota.
• 1–2 years of MDS experience preferred.
• Strong knowledge of RAI, ICD-10 coding, and PDPM.
• Experience in a skilled nursing facility (SNF) or long-term care setting.
• Strong organizational skills and attention to detail.
• Excellent communication and teamwork abilities.

Responsibilities

• Coordinate and complete MDS assessments including OBRA and PPS.
• Ensure accuracy and timeliness of MDS documentation and submissions.
• Collaborate with interdisciplinary team to develop care plans.
• Monitor case mix index (CMI) and identify optimization opportunities.
• Ensure CMS and Minnesota state regulatory compliance.
• Participate in care plan meetings with residents and families.
• Assist with audits, surveys, and quality improvement.
• Stay updated on regulatory changes and reimbursement updates.

Employment Type: Full-Time

Position Summary:
Southview Acres Healthcare Center is seeking a detail-oriented and experienced MDS Coordinator to join our clinical leadership team. The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations, ensuring accurate reimbursement, and supporting high-quality resident care.

Key Responsibilities

• Coordinate and complete MDS assessments (including OBRA, PPS, and any applicable state-specific assessments)
• Ensure accuracy and timeliness of all MDS documentation and submissions
• Collaborate with interdisciplinary team members to develop and update comprehensive care plans
• Monitor case mix index (CMI) and identify opportunities for optimization
• Ensure compliance with CMS guidelines and Minnesota state regulations
• Participate in care plan meetings and communicate effectively with residents and families
• Assist with audits, surveys, and quality improvement initiatives
• Stay up to date on regulatory changes and reimbursement updates

Qualifications
