Job summary

Heartland Hospice is seeking an After Hours/On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. You will lead direct patient care, serve as the primary liaison among patients, families, and physicians, and ensure timely symptom management in the patients' homes.

You will document thoroughly, collaborate with the daytime team, and participate in quality improvement efforts while upholding our compassionate care standards and core values.

Qualifications

• Current RN license in the state of requested employment.
• 1+ years of nursing experience.
• A hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and the ability to provide support in emotionally intense settings.
• Comfortable providing care in patients’ homes, facilities, or wherever they reside.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs after hours and holidays as the primary liaison for patients, families, physicians, and the hospice team.
• Provide direct patient care and support during evenings, nights, weekends, and holidays.
• Accurately document visits and communications in the medical record.
• Coordinate with the daytime team to ensure continuity and quality of care.
• Participate in QA and performance improvement programs.
