Job summary

Sage Hospice Primary & Palliative Care is seeking a compassionate PRN On-Call Registered Nurse (RN) to join our hospice team in the Tempe, AZ area. You will provide skilled nursing care at patients’ homes, coordinate with an interdisciplinary team, and support families navigating end-of-life care.

The role requires an RN license, CPR certification, and experience in hospice or home health. Strong communication and computer skills are essential for a smooth, patient-centered care experience.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient with computers; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to patient and family.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
