Job summary

Sage Hospice Primary & Palliative Care in Glendale, AZ is seeking a passionate PRN On-Call Registered Nurse (RN) to join our hospice team. You will administer skilled nursing care in patients' homes, coordinate with the interprofessional team, and support families with compassionate, evidence-based care.

Requires an active RN license in Arizona, CPR certification, and at least two years of nursing experience in a clinical setting; hospice or home health is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient skill using a computer and MS Office is preferred.
• Strong interpersonal skills and ability to support patients and families.
• Nursing practice skills with resilience to family emotional stress.

Responsibilities

• Primary functions: administer skilled nursing care for clients in their home.
• Coordinate care with the interdisciplinary team, patient/family, and referrer.
