Job summary

Reserve Care, formerly Hospice of the Western Reserve, is seeking an On-Call Nurse (RN or LPN) for weekend shifts in Willoughby Hills, OH. You will provide after-hours nursing care in patients' homes, assessing needs and coordinating with physicians.

This role requires strong communication, independence, and empathy to support patients and families in challenging times. The position offers flexible weekend hours, the opportunity to utilize nursing skills, and a meaningful impact on end-of-life

Qualifications

• High School Diploma or equivalent required.
• Graduate of an accredited nursing program; Bachelor's preferred.
• Licensed RN in Ohio.
• Experience in pain and symptom management, oncology, or mental health preferred.
• Ability to communicate effectively, orally and in writing; strong teamwork.

Responsibilities

• Assess physical, psychosocial and spiritual needs of the patient and family (RN only).
• Provide intermittent skilled nursing services under physician direction.
• Monitor and evaluate pain and symptoms; inform patient and family.
• Liaise with physicians; provide information based on patient needs.
• Provide emotional support to patient and family.
• Refer patients to other services when appropriate.
• Provide home health teaching.
• Make visits to patient homes, including at the time of death.
• Maintain and return documentation promptly.
• Communicate with patients, family, and health professionals with confidentiality.
• Relay pertinent information to team members.
• Participate in team meetings and trainings.
• RNs to participate in triage coverage.
• Perform other duties as assigned.
