Solara Hospice is seeking a dedicated Hospice RN Case Manager to coordinate and deliver comprehensive nursing care for patients in Eastern Massachusetts. This role supports patients and families through the end-of-life journey with compassionate, interdisciplinary coordination.

Requirements include an active Massachusetts RN license, 1–2 years of nursing experience preferred, strong clinical and communication skills, and the ability to work independently with reliable transportation.
