Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) to join our team. This role provides skilled nursing care for patients in their homes, coordinates treatment with an interdisciplinary team, and communicates effectively with patients, families, and referring agencies.

Qualifications include an associate or bachelor’s degree in nursing, current RN license in Colorado, CPR certification, and at least two years of clinical RN experience.

Qualifications

• Associate or bachelor’s degree in Nursing required.
• Current RN license in Colorado and CPR certification.
• Two years of RN experience in a clinical care setting; hospice or home health experience preferred.
• Current driver’s license may be required.

Responsibilities

• Administer skilled nursing care for clients in their homes, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.
