Job summary

Three Oaks Hospice and its network of sister companies are seeking a compassionate PRN On-Call Registered Nurse to provide skilled nursing care in patients’ homes, coordinate with the interdisciplinary team, and support families in times of need.

We value clinical excellence, patient advocacy, and reliability. Qualifications include an RN license, CPR certification, 2 years RN experience, and proficiency with Microsoft Office; hospice/home health experience preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer is required; Microsoft office suite experience is preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.

Responsibilities

• Administer skilled nursing care at clients’ homes, coordinate care with the interdisciplinary team, patient/family and referring agency; and assume the responsibility for coordination of care.
