Job summary

Qualifications

• Must have a State Certified RN license.
• Experience in hospice or home health care, REQUIRED.
• Associate's Degree in Nursing required.

Responsibilities

• Assumes responsibility for a patient visit or on-call schedule including assessment, planning, implementing and evaluating nursing processes.
• Provides hospice nursing care focusing on pain control and symptom management.
• Initiates communication with attending physicians, hospice staff, and other agencies to coordinate care.
