Job summary

Paradigm Health in Indiana is seeking a Full-time Hospice RN On-Call to cover evenings and overnight shifts in a responsive, patient-centered environment. The role requires a valid Indiana RN license and at least 1 year of RN experience with hospice preferred.

Mileage reimbursement, PTO, health/dental/vision, and a 401k match are offered as part of a competitive compensation package. Territory coverage includes response within an hour of on-call needs.

Qualifications

• One year Hospice experience preferred
• 1 year of RN experience required
• Licensed Registered Nurse in State of Indiana
• Current driver's license
• Must have reliable transportation and insurance

Responsibilities

• Performs physical and psychosocial assessment on all hospice patients
• Ability to adequately assess and reassess pain
• Educates the patient and family regarding pain management
• Responds to the physical, mental, social and emotional needs of terminally ill patients and their families
• Assesss patient’s condition, initiates plan of care, re-evaluates and updates as necessary
• Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate
• Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.
