Job summary

Paradigm Health in Indiana is seeking a full-time Hospice RN On-Call to provide compassionate end-of-life care across our region. You will perform patient assessments, manage pain, and collaborate with a multidisciplinary team to ensure high-quality, personalized care.

The role requires an Indiana RN license, one year hospice and RN experience, a valid driver's license and reliable transportation. We offer competitive pay, mileage reimbursement, PTO, health benefits and a supportive team.

Responsibilities

• Initiate and update plans of care; collaborate with physicians.
• Initiate case conferences with the multidisciplinary team.

Job Description

• Full-time Hospice RN On-Call
• Friday 5:00pm-11:00 pm; Saturday/Sunday 8:00 am - 11:00 pm
• Territory: We respond to On-call needs within an hour
• Performs physical and psychosocial assessment on all hospice patients.
• Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
• Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
• Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
• Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
• Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.
