Job summary

Paradigm Health is seeking a dedicated Hospice RN On-Call in Indiana. This full-time role covers evening to early-morning shifts, with the team responding to on-call needs within an hour.

The RN will perform comprehensive patient assessments, manage pain, and coordinate with physicians to ensure quality end-of-life care for hospice patients. Requirements include Indiana RN licensure, 1 year RN experience, and a valid driver's license with reliable transportation.

Qualifications

• One year Hospice experience preferred.
• 1 year RN experience required.
• Licensed Registered Nurse in State of Indiana.
• Current driver's license.
• Reliable transportation and insurance.

Responsibilities

• Perform physical and psychosocial assessment on hospice patients.
• Assess and reassess pain; educate patient and family on pain management.
• Respond to physical, mental, social, and emotional needs of terminally ill patients and families.
• Initiate plan of care, re-evaluate and update as necessary.
• Obtain physician orders and collaborate with physician as needed.
• Initiate case conferences to discuss multidisciplinary team responsibilities and pain management.
