Job summary

Paradigm Health in Indiana is seeking a full-time Hospice RN on-call to join our patient-focused team serving Kokomo and surrounding areas. You will perform physical and psychosocial assessments, manage pain, coordinate with physicians, and respond promptly to on-call needs within an hour.

Qualifications

• One year hospice experience preferred.
• RN license in Indiana is required.
• Current driver's license and reliable transportation.

Responsibilities

• Performs physical and psychosocial assessment on all hospice patients.
• Assess and manage pain; educate patient and family on pain control.
• Respond to on-call needs within an hour; initiate plan of care.
• Obtain physician orders and collaborate with physicians as needed.
• Initiate case conferences to discuss team responsibilities and care planning.

Job Description

• Full-time Hospice RN On-Call

• Monday-Thursday 5:00 pm - 8:00 am
• Territory: We respond to On-call needs within an hour
• Performs physical and psychosocial assessment on all hospice patients.
• Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
• Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
• Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
• Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
• Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.

Requirements

• One year Hospice experience preferred
• 1 year of RN experience required
• Licensed Registered Nurse in State of Indiana
• Current driver's license
• Must have reliable transportation and insurance
