Job summary

Paradigm Health in Indiana is seeking a full-time Hospice RN On-Call to provide compassionate end-of-life care across our region. You will perform patient assessments, manage pain, and collaborate with a multidisciplinary team to ensure high-quality, personalized care.

Qualifications

• One year hospice experience required.
• One year of RN experience required.
• Licensed Registered Nurse in Indiana.
• Current driver's license.
• Reliable transportation and insurance.

Responsibilities

• Perform physical and psychosocial assessments on hospice patients.
• Assess and manage pain, educate patients and families.
• Respond to the physical, mental, social, and emotional needs of terminally ill patients and families.
• Initiate and update plans of care; collaborate with physicians.
• Initiate case conferences with the multidisciplinary team.
