Job summary

Visiting Nurse Health System is hiring an RN Case Manager for Hospice, coordinating care for a home-based caseload in the Atlanta region. You will assess, plan, implement, monitor, and evaluate care plans to achieve optimal patient outcomes.

The role requires strong clinical skills, education, and the ability to educate families while navigating Medicare/Medicaid guidelines. Expect a 7 day on/7 day off schedule across North & South Atlanta with travel as needed.

Qualifications

• Graduated from an accredited nursing program (Diploma, ADN, or BSN).
• Hold Georgia State Board of Nursing licensure and CPR certification.
• Experience in SNH/home health or hospice is preferred.

Responsibilities

• Coordinate care plans for a patient caseload and ensure quality standards.
• Develop, modify, and evaluate care plans with the interdisciplinary team.
• Educate patients and families on disease processes and self-management.
• Submit OASIS/documents and visit notes in a timely manner.
• Communicate status changes clearly to patients and team members.
