Visiting Nurse Health System is seeking a Registered Nurse Case Manager specialized in hospice care for full-time positions in Carrollton, GA. You will coordinate comprehensive care for patients, assessing and implementing tailored care plans while providing education to families.

This role values compassion and effective communication as you connect with patients in their homes, ensuring their needs are met while supervising additional nursing staff.

Qualifications

• Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred).
• Hold current licensure by the Georgia State Board of Nursing and valid CPR certification.
• Prior home health experience if applying as a PRN staff member.
• Knowledge of Medicare, Medicaid, and home health care regulations.
• Proof of decision-making, time management, and interpersonal skills.

Responsibilities

• Develop and implement individualized care plans for patients.
• Modify care plans as patient conditions change.
• Provide patient and family education on self-management strategies.
• Document changes in patient status appropriately.
• Supervise LPNs and HHAs according to policy.
• Participate in SOC/ROC conference calls and team conferences.
