Job summary

Infinity Hospice Care LLC is seeking an RN Case Manager to perform admissions and provide nursing care for patients in their homes. You will coordinate care with the Interdisciplinary Team, patients, families, and referring agencies, ensuring comprehensive and compassionate hospice services.

Requirements include an active RN license and 2 years of clinical RN experience with at least 1 year in hospice. The role involves travel to patient homes, on-call duties, and adherence to OSHA infection

Qualifications

Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.

Responsibilities

Assess hospice patients using Nursing Assessment and LCD criteria to identify needs.
Develop and document the plan of care in correlation with the assessment.
Provide nursing care with infection control standards (OSHA).
Coordinate with Interdisciplinary Group/Team to meet patient/family needs.
Communicate with attending physicians and hospice physicians involved in care.
Submit all required paperwork in a timely manner.
Participate in hospice in-service programs and QAPI involvement.
Meet productivity standards and perform on-call responsibilities.
Communicate effectively with patients, families, team and other health professionals.
Travel to patient homes; maintain valid auto liability insurance.
