Job summary

Qualifications

• Two years RN experience in a clinical care setting.
• One year full-time hospice experience.
• Graduate of an accredited nursing program.
• Current state RN license and driver's license.

Responsibilities

• Assess hospice patients and identify physical, psycho-social, and environmental needs.
• Develop and document the plan of care in correlation with the assessment.
• Provide nursing care utilizing infection control measures.
• Work with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending physicians and hospice physicians involved in care.
• Submit all required paperwork timely.
• Participate in hospice in-service programs and QAPI activities.
• Meet productivity standards and on-call responsibilities.
• Provide effective communication to patients, families, and health professionals.
• Maintain reliable transportation for home visits.
