Job summary

Infinity Hospice Care LLC seeks an RN Case Manager to perform admissions, provide bedside nursing care in patients' homes, and coordinate with the interdisciplinary team to meet patient and family needs.

The role requires developing and documenting care plans, ensuring infection control, and timely paperwork submission while collaborating with physicians and other professionals. On-call duties and home visits are essential aspects of the position.

Qualifications

• Two years experience as a Registered Nurse in a clinical care setting.
• One year full-time hospice experience.
• Graduation from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse; Current driver’s license

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria to identify needs.
• Develop and document the plan of care in correlation with the Assessment.
• Provide nursing care utilizing infection control measures.
• Work with Interdisciplinary Group/Team to ensure patient/family needs are met.
• Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
• Timely submission of all required paperwork.
• Demonstrates commitment and professional growth by participating in hospice-sponsored in-service programs and maintaining or improving competency. Participates in the Hospice’s QAPI program.
• Promotes the Hospice’s philosophy and administrative policies.
• Meets productivity standards.
• Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
• Provides effective communication to patients, their family members, team members, and other health care professionals.
