Job summary

Affinity Hospice is seeking a Registered Nurse to serve as a case manager, directing hospice care within patients' homes and communities. The role emphasizes community health education, coordination with the interdisciplinary team, and ensuring a comprehensive care plan.

The position requires a nursing license, CPR certification, and at least two years of RN experience in clinical care, home health, or hospice settings. Candidates must be able to lift patients and drive as needed.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State RN license and CPR certification.
• Minimum of two years in clinical care, home health or hospice setting preferred.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician and family.
• Implement and document patient plan of care.
• Provide care using infection control measures (OSHA).
• Ensure continuity of quality patient care with proper documentation.
