Job summary

Vitality Hospice in Pennsylvania seeks an On-Call Registered Nurse (RN) to provide compassionate after-hours hospice care, respond to patient needs, manage symptoms, support families, and ensure comfort during evenings, nights, and weekends.

You will work with an experienced interdisciplinary team, perform nursing assessments, document in the EMR, coordinate care, and comply with Medicare and hospice standards while delivering exceptional patient service.

Qualifications

• Must hold a Pennsylvania RN license.
• Minimum 1 year of RN experience (Hospice, Home Health, Oncology, ICU, ER, or Med-Surg preferred).
• Hospice experience preferred, but training available.

Responsibilities

• Respond to after-hours patient needs and symptom management.
• Perform nursing assessments and develop appropriate interventions.
• Provide compassionate education and emotional support to patients and families.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Complete timely, accurate documentation in the EMR.
• Maintain compliance with Medicare, state regulations, and hospice standards.
• Participate in rotating on-call coverage while providing exceptional customer service.
