Job summary

Vitality Hospice is seeking an On-Call Registered Nurse (RN) to provide compassionate after-hours hospice care in the Pennsylvania territory. The role involves responding to patient needs, symptom management, and supporting families during evenings, nights, and weekends.

You will work with an experienced interdisciplinary team to deliver exceptional hospice care, participate in rotating on-call coverage, and maintain high standards for documentation and compliance.

Qualifications

• Current Pennsylvania RN license is required.
• Minimum of 1 year RN experience (Hospice/Home Health/Oncology/ICU/ER/Med-Surg preferred).
• Hospice experience preferred, but training available for the right candidate.

Responsibilities

• Respond to after-hours patient needs and symptom management.
• Perform nursing assessments and develop interventions.
• Educate patients and families with compassion.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Complete timely documentation in the EMR.
• Maintain compliance with Medicare, state regulations, and hospice standards.
• Participate in rotating on-call coverage.
