Vitality Hospice in Pennsylvania is seeking an On-Call Registered Nurse (RN) to provide compassionate after-hours hospice care. You will respond to patient needs, manage symptoms, support families, and ensure comfort during evenings, nights, and weekends, working with an experienced interdisciplinary team.

Qualifications

• Current Pennsylvania Registered Nurse (RN) License required.
• Minimum of 1 year of RN experience (Hospice, Home Health, Oncology, ICU, ER, or Med-Surg experience preferred).
• Hospice experience preferred, but we're willing to train the right candidate.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and compassionate bedside manner.
• Ability to work independently while collaborating with the clinical team.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Respond to after-hours patient needs and symptom management.
• Perform nursing assessments and develop appropriate interventions.
• Provide compassionate education and emotional support to patients and families.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Complete timely, accurate documentation in the EMR.
• Maintain compliance with Medicare, state regulations, and hospice standards.
• Participate in rotating on-call coverage while providing exceptional customer service.
