Job summary

Vitality Hospice & Palliative Care in Pennsylvania is seeking a compassionate Registered Nurse (RN) for a full-time On-Call schedule with 7 days on / 7 days off. You will provide after-hours hospice care, maintain clinical excellence, and support patients and families in the Lehigh Valley area.

Responsibilities include symptom management, assessments, and coordinating care with an interdisciplinary team. A PA RN license and 1 year RN experience are required, hospice experience preferred but

Qualifications

• Must hold PA RN license.
• Minimum 1 year RN experience.
• Hospice, Home Health, Oncology, ICU, ER or Med-Surg experience preferred.
• Hospice experience is preferred but training available.
• Strong assessment and critical-thinking skills.
• Compassionate and confident communication.

Responsibilities

• Respond to after-hours patient needs and changes in condition.
• Provide symptom and pain management.
• Perform nursing assessments and determine interventions.
• Make patient visits when clinically necessary.
• Educate and support patients and families.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Complete timely and accurate EMR documentation.
• Ensure continuity of care during on-call week.
• Maintain compliance with Medicare, state, and hospice standards.
