Job summary

Affinity Health Management is seeking a Hospice On Call RN to respond to census, handle referrals and community inquiries outside office hours, and provide urgent clinical and psychosocial interventions to patients and families.

The role emphasizes independent assessment in crisis, pain and symptom management, collaboration with the Hospice Medical Director and team, and thorough documentation, with nights, weekends, and holidays required.

Qualifications

• Excellent observation, verbal and written communication skills.
• Problem solving skills and nursing skills per competency checklist.
• Maintains current driver’s license and automobile insurance, providing own transportation.

Responsibilities

• Responds to after-hours inquiries and urgent patient/family needs.
• Performs urgent physical and psychosocial assessments.
• Provides pain and symptom management and nursing care.
• Documents care thoroughly and timely.
• Collaborates with physicians and the IDG for continuity of care.
• Coordinates with vendors for DME, supplies and medications after-hours.
