The successful candidate will hold an active RN license, have at least 1 year of clinical experience (hospice/home health preferred), and demonstrate strong assessment and communication skills.

Qualifications

• Active RN license
• 1 year of clinical care experience
• Hospice or home health experience preferred
• 50% travel required
• Valid driver’s license and reliable transportation

Responsibilities

• Assess hospice patients and families to identify physical, psychosocial, and environmental needs.
• Communicate significant findings and changes to the Clinical Supervisor, physician, and care team.
• Counsel, instruct, and include patient and family in the Interdisciplinary Plan of Care.
• Teach hospice philosophy and pain management to staff in alternate care settings.
• Provide bereavement assessment and support at time of death or aftercare.
• Perform after-hours/on-call nursing duties and coordinate with on-call schedule.
