Job summary

Elara Caring in Shawnee, Oklahoma, is seeking an After-Hours/On Call Registered Nurse to join our hospice/home health team. You will assess patients and families, monitor conditions, and coordinate with the physician and care team to ensure comfort and safety in the home and facility settings.

The role requires a current RN license, graduation from an accredited nursing program, and at least one year of clinical nursing experience; home health experience is preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
