Job summary

Elara Caring is seeking an After-Hours/On Call Registered Nurse to support hospice and home health patients in the Oklahoma area. You will assess patients, respond to changes, and collaborate with the care team to adjust the Plan of Care.

This role requires an active RN license, at least one year of nursing experience, ability to lift up to 50 pounds, and a valid driver’s license with reliable transportation to travel between patient homes.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate/ Diploma/ ADN/ BSN).
• 1+ year of clinical nursing experience.
• Home health or hospice experience preferred.
• Ability to lift/move up to 50 pounds.
• Valid driver’s license or reliable transportation.

Responsibilities

• Assess hospice patients and families to identify needs.
• Monitor patient condition and report changes to the care team.
• Educate patients and families on Plan of Care.
• Provide pain and symptom management education to staff.
• Provide bereavement support to families.
