Job summary

Qualifications

• Graduation from an accredited school of nursing and Michigan RN licensure.
• Minimum of one year Hospice RN experience.
• Understanding of Hospice concept of care and knowledge of responses to hospice and terminal illness.
• Valid driver's license and acceptable driving record.
• Hospice certification preferred.
• Strong interpersonal skills to present a positive image of Hospice.
• Ability to be flexible in adapting policies to meet Hospice needs.

Responsibilities

• Assess after-hours patient needs per hospice care plan.
• Coordinate with the Interdisciplinary Team to provide patient/family care.
• Maintain continuity of care by ensuring patient needs are met.
• May perform on-call triage or home visits after hours.
