Job summary

Elara Caring is seeking a Hospice Registered Nurse for after-hours/on-call Saturday-Monday 8 AM–9 PM, full-time, in Michigan. We care for patients in their homes, working with a compassionate Interdisciplinary Team to deliver high-quality end-of-life care.

Qualifications

• Graduation from an accredited nursing program (Certificate/Diploma/AS/BSN).
• Current State RN license.
• Minimum 1 year clinical care experience.
• Hospice or home health experience preferred.
• Valid driver’s license and reliable transportation.
• Ability to travel up to 50%.

Responsibilities

• Assess hospice patient and family needs and document findings.
• Assess data on physical, psychosocial, and environmental needs.
• Communicate changes in health condition to care team.
• Counsel patient, family, and facility per Interdisciplinary Plan of Care.
• Teach hospice philosophy and pain management to staff.
• Provide bereavement support after death.
