Job summary

Amara Hospice Care is seeking a Registered Nurse (RN) Case Manager to join our team serving Saginaw and surrounding areas. You will conduct initial patient and family assessments and develop care plans in collaboration with the healthcare team.

The ideal candidate holds a Michigan RN license, is willing to travel the area, and has hospice experience. Reliable transportation and insurance are required; schedule is Mon–Fri, 8:30am–4:30pm, with on-call rotation offered for extra pay.

Qualifications

• Michigan RN license in good standing required.
• Hospice experience preferred and willing to travel the service area.
• Reliable transportation and valid insurance needed.

Responsibilities

• Complete initial patient and family assessments to determine hospice care needs.
• Re-evaluate nursing needs and update care plans with the team.
• Coordinate with healthcare team, patient, and family for continuous care.
• Counsel patients and families on end-of-life needs and care options.
• Initiate appropriate nursing procedures and report changes to physician.
• Supervise Hospice Aides and LPNs; participate in on-call rotation (additional pay).
