Job summary

Amara Hospice Care LLC in Saginaw, MI is seeking a Registered Nurse Case Manager to support case management and home-based hospice visits across Saginaw and surrounding areas. You will assess patients, develop and adjust care plans, and coordinate with the care team.

The ideal candidate holds a Michigan RN license, is willing to travel, and can supervise aides and LPNs. A reliable vehicle and insurance are required; schedule is Mon-Fri 8:30 am–4:30 pm with on-call rotation optional.

Qualifications

• Current Michigan RN license in good standing.
• Willingness to travel within Saginaw area and surrounding communities.
• Reliable transportation and valid insurance are required.
• Hospice experience preferred.

Responsibilities

• Complete initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs and adjust care plans.
• Collaborate with the healthcare team to direct the work and provide coordinated care.
• Counsel patients and families in meeting end-of-life needs.
• Initiate appropriate preventative nursing procedures.
• Communicate patient needs to the physician and report changes in condition.
• Supervise Hospice Aides and LPNs.
• Participate in On-Call rotation when needed (extra pay).
