Vitality Hospice & Palliative Care is seeking a compassionate Registered Nurse (RN) for a full-time On-Call position with a 7 Days On / 7 Days Off schedule in Pennsylvania. You’ll provide after-hours hospice care, manage symptoms, and coordinate with the interdisciplinary team while enjoying extended time off every other week.

The role requires PA RN license, at least 1 year of RN experience (hospice or related fields preferred), and the ability to work independently with strong clinical

Qualifications

• Current Pennsylvania RN license.
• Minimum 1 year of RN experience.
• Hospice, Home Health, Oncology, ICU, ER, or Med-Surg experience preferred.
• Hospice experience preferred but training available.
• Strong assessment and critical-thinking skills.
• Ability to work independently and make sound clinical decisions.
• Valid driver’s license and reliable transportation.

Responsibilities

• Respond to after-hours patient needs and changes in condition.
• Provide symptom and pain management.
• Perform nursing assessments and determine interventions.
• Make patient visits when clinically necessary.
• Educate and support patients and families.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Complete timely EMR documentation.
• Ensure continuity of care throughout on-call week.
• Maintain compliance with Medicare, state, and hospice standards.

What You'll Do

As our On-Call RN, you'll provide compassionate after-hours hospice care and be a reassuring presence for patients and families when they need support most.

Your responsibilities will include:
