Job summary
Continuum Hospice is seeking a Full Time On-Call Registered Nurse (RN) to provide compassionate hospice care for patients and families in Santa Cruz and surrounding areas. The role emphasizes assessment, coordination of care, and preventative support during critical moments.
On-call schedule rotates 7 days on/off with specific weekend hours; the team offers a comprehensive benefits package and ongoing professional development opportunities to support growth and retention.
Qualifications
- RN license in good standing
- Bachelor's degree in nursing from NLN-accredited program
- 1 year of nursing experience within last 3 years
- Hospice/home health experience preferred but not required
- Computer proficient with EMR systems
- Current CPR certification
Responsibilities
- Respond to a range of patient needs from urgent to routine, providing unwavering support
- Conduct comprehensive assessments of physical, emotional, social, and spiritual needs
- Maintain accurate documentation per agency and regulatory standards
- Support families by sharing knowledge and guiding care for terminally ill loved ones
