Job summary

Continuum Hospice & Palliative Care California is seeking a Full Time On-Call Registered Nurse to provide compassionate, patient-centered hospice care in the Santa Cruz area. You will assess needs, coordinate care, and support families during critical moments in the home.

The role requires an active RN license, BSN, and EMR proficiency, with CPR certification. Hospice experience is preferred; on-call shifts rotate with the team.

Qualifications

• Current unencumbered RN license.
• BSN or NLN-accredited program.
• 1 year professional nursing experience within last 3 years.
• Hospice/home health experience preferred but not required.
• Computer proficiency with EMR
• CPR certification must be current.

Responsibilities

• Respond to patient needs from urgent to routine in home setting.
• Conduct comprehensive assessments of physical, emotional, social, and spiritual needs.
• Maintain accurate documentation per agency and regulatory standards.
• Support families by sharing knowledge and guiding care at end of life.
