Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) to join our compassionate hospice team. The role focuses on delivering skilled nursing care to patients in their homes, coordinating with an interdisciplinary team, families, and referring agencies to ensure seamless care.

Qualified candidates hold an RN license, CPR certification, and a degree in nursing, with hospice or home health experience preferred. Excellent communication and coping skills with families are essential.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to patient and family.
• Demonstrate skill in nursing practice and tolerance of family emotional stress.

Responsibilities

• Administer skilled nursing care for clients in their place of residence and coordinate care with the interdisciplinary team and referring agency.
• Coordinate patient care with families to ensure continuity and quality of care.
