Job summary

AdvisaCare is seeking an On-Call Hospice Nurse to provide compassionate home-health care across the Macomb, Oakland, and Wayne County areas. Responsibilities include triaging patient calls, making visits, performing tuck-ins, and handling admissions on a 7 days on / 7 days off schedule.

We want a current Michigan RN license with hospice experience or willingness to learn, plus an empathetic, organized, and adaptable professional ready to join our Hospice Family.

Qualifications

• Current Michigan RN license in good standing.
• Hospice experience preferred but not required.
• Empathetic approach to patients and caregivers.
• Ability to participate in field visits and on-call duties.

Responsibilities

• Triage patient calls and assess needs.
• Make patient visits and provide admissions as needed.
• Perform tuck-ins and coordinate with the care team.

We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family servicing the Macomb. Oakland and Wayne County areas!

Our On-Call Nurse will Triage patient calls, make patient visits, preform tuck-ins, and do Admissions as needed.

Requirements

• Current Michigan RN License in good standing
• 1 year hospice experience preferred but not required
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient's status and needs on a on call basis
