Job summary

Good Shepherd Hospice is seeking an experienced hospice LPN to work on call in Newton and surrounding communities. The On Call Nurse is responsible for after-hours nursing needs, responding to calls promptly and visiting patients as needed.

Responsibilities include coordinating with the RN Case Manager, maintaining patient records, and educating families. This role requires strong organizational skills and a commitment to compassionate hospice care.

Qualifications

• Minimum of one year of hospice, home health or geriatric care experience preferred
• Must maintain confidentiality and integrity including patient records
• LPN license and current state license required
• Valid driver’s license

Responsibilities

• Receives patient assignments from the Patient Care Manager
• Coordinates with RN Case Manager, hospice staff and other agencies to coordinate care
• Maintains regular communication with attending physician and family
• Maintains up-to-date patient records and Medicare paperwork as required
• Provides caregiver education on medication administration, personal care and mobility
• Supports 24/7 coverage to patients and families
• Participates in care planning and hospice team meetings
