Qualifications

Responsibilities

• Receive patient assignments from the Patient Care Manager.
• Coordinate with RN Case Manager and hospice staff to optimize patient care.
• Communicate with families and provide bereavement support as needed.
• Maintain up-to-date patient records and Medicare paperwork.
• Teach caregivers on medication administration, personal care, and safety.
• Participate in care plan meetings and team conferences.
• Provide 24/7 coverage for patients and families as needed.
• Contribute to pain management and hospice care education.
