Job summary

Good Shepherd Hospice on-call Registered Nurse serves patients and families in Newton and surrounding communities, performing nursing assessments, planning and care to maximize comfort and health according to the Interdisciplinary Plan of Care.

The role includes on-call duties with every other weekend, with hospice experience preferred and required RN licensure in the state. Strong integrity, teamwork, and communication are essential to provide compassionate care.

Qualifications

• Must be a current RN licensed to practice in the state.
• Hospice, home health, or geriatrics experience preferred.
• Demonstrated integrity and ability to maintain confidentiality.
• Team player, following care plans.
• Valid driver\'s license.

Responsibilities

• Receives patient assignments and coordinates care with the care team.
• Maintains communication with attending physicians, hospice staff, and agencies.
• Documents data for the Plan of Care and updates records per policy.
• Instructs patients and caregivers on medications and care tasks.
• Provides 24/7 coverage to patients and families as needed.
• Attends team meetings and care plan reviews.
