Job summary

Qualifications

• High School Graduation or GED with 3 years direct mental health/community services, or an Associate/Bachelor's in Social Services.
• At least 1 year of direct crisis intervention experience with screening and assessments.
• Sensitivity to multi-cultural populations and issues.
• Must be at least 18 years of age.
• CPR, CPI, and First Aid certifications on hire or within 60 days and maintained thereafter.
• Valid driver's license and personal vehicle insurance in your name; willingness to use company vehicle.
• Willingness to use personal vehicle for meetings; mileage reimbursed per IRS rates.

Responsibilities

• Provide screening, assessment, and primary support as a case manager to link needed services for members and their families.
• Support clients in crisis to access resources and connect to appropriate services.
• Operate under direct supervision and coordinate with the multidisciplinary team.
• Attend to transportation needs for clients using company or personal vehicle as required.
